Polypharmacy is common in patients with chronic conditions, increasing the risk of adverse drug reactions (ADRs), drug interactions, medication nonadherence, falls and hospital admissions.1 Additionally, it increases the possibility of “prescribing cascades” where additional medications are prescribed to treat misinterpreted ADRs.2 Interactions with over-the-counter medications may also occur with many widely used prescription medications.
